Rudraprayag District Disaster Management Officer Nandan Singh Rajwar told PTI that the accident took place above the forests of Gaurikund amid poor visibility due to bad weather.
Chief of Air Staff Air Chief Marshal VR Chaudhari and Air Marshal Manvendra Singh who headed the tri-service investigation into the crash apprised Defence Minister Rajnath Singh on the findings of the probe on Wednesday.
'The accident was a result of entry into clouds due to unexpected change in weather conditions in the valley. This led to spatial disorientation of the pilot resulting in Controlled Flight into Terrain (CFIT),' it stated.
HAL helicopters will be equipped with an 'Obstacle Avoidance System' that warns pilots of any obstacles in their flight path.
